东汉末年，外戚专权，宦官干政，**。加之天灾人祸，因此民不聊生。公元184年，张角兄弟揭竿而起，发动了黄巾起义。起义军声势浩大，各州县出榜招兵，乱世来临。在涿州，一位以贩席为生却胸怀大志的英雄人物刘备认为出人投地的时机已至。在招兵的榜文前，刘备结识了关羽、张飞二人。刘备以皇室后裔自居，以其独特的魄力说服关、张二人与自己一起创立大业。三人结为兄弟并投军，参与了剿灭黄巾之战。
后来，在各路官军合力镇压之下，黄巾起义失败。刘备获战功被封为安喜县尉。朝庭派来淘汰官吏的督邮来县，借机索要贿赂。三兄弟得知后怒鞭督邮，弃官而去。后又多次辗转于扫除黄巾余*的战场，数立战功后任平原令。
公元189年，汉灵帝死，大将军何进介入皇权之争。为除干政的宦官一*，何进召董卓带西凉军进京。董卓进入洛阳时，何进已被宦*暗杀，于是董卓收何进之众，势力大盛，据兵擅政，废少帝而立献帝，自己进位为相国，权倾朝野。
校尉袁绍与曹操召集天下诸侯起兵共讨董卓，刘备三兄弟也随北平公孙瓒前来会盟。长沙孙坚作先锋与大将华雄交战，却因盟军各怀异心而失利。刘备三兄弟迎难而上，不仅斩了华雄，更是在虎牢关大败有天下第一猛将之称的吕布。盟军声势浩大，董卓被迫携汉帝迁都长安。经此一役，颇具战略目光的曹操开始注意与众不同的刘关张三人，特别是温酒斩华雄的关羽。
盟军进入洛阳后，矛盾日渐激化，不久诸侯解盟，甚至彼此火并。董卓因此更加残暴，民心丧尽。司徒王允用歌妓貂蝉设下连环计，终于使董卓与吕布反颜。公元192年，董卓为王允、吕布合谋诛杀。
奉立天子·江东立业(第10至19回)
董卓死后，中原大乱，董卓残部与军阀之间争权夺势，黄巾余*与山贼也趁势作乱。曹操父亲曹嵩在徐州遇害，曹操因此以报仇为名攻打徐州。徐州太守陶谦不敌曹操，向外埠求援。刘备三兄弟借此机会带兵来到徐州，公孙瓒也派出少年将军赵云相助刘备。而曾经对曹操有恩的陈宫不满曹操屠城的残忍，说服吕布在乱世中建立功业，二人率军袭击了山东。曹操被迫回战吕布。公元194年，陶谦死，刘备接领徐州。
曹操与吕布几番苦战，在定陶大破吕布。吕布走投无路，来徐州投了刘备。后来吕布趁刘备与袁术交战之时攻占了徐州，刘备被迫驻军小沛。
汉献帝被董卓残部李傕、郭汜追杀的消息传来。曹操知时机已至，率军救驾，剿灭乱军，于公元196年将献帝迁至许都(后改许昌)，取得了“携天子以令诸侯”的*优势。不久又收降了张绣，实力大增。
公元191年，江东孙坚在与荆州刘表的交战中身亡，其子孙策势单力孤，只能率残部依俯于淮南袁术，却常思创立基业。数年后，孙策向袁术借兵征讨江东，在寿春遇慕名而来的一代儒将周瑜。周瑜与孙策一见如故，两人结为兄弟。在周瑜的帮助下，孙策逐渐平定江南六郡，终于以其非凡的勇略与魄力，奠定了日后三分天下有其一的吴国的基业。
公元197年，袁术称帝，曹操联合吕布、孙策征讨袁术，袁术大败而逃。吕布为求州牧，派出陈登为使与曹操相见，却不知陈登素来痛恨吕布，与曹操相见后，反而成为了曹军的内应，离间吕布与陈宫的关系。渐渐地，吕布与陈宫失和，一意孤行而中曹操之计。公元198年，曹操与刘备联合，依谋士郭嘉之计，终于大破吕布，收降了大将张辽。吕布与陈宫在下邳被曹操处死。
袁曹对峙·孙权继位(第20至29回)
曹操收复徐州后，势力再次扩大。因收复徐州有功，曹操官晋丞相，权倾朝野。刘备随曹操入许都见献帝，献帝拜刘备为皇叔，授左将军，借以培植势力与曹操对抗。刘备私下与西凉马腾等人结为反曹联盟。梅熟之际，曹操约刘备赏梅，共论天下人物。曹操蔑视群雄，独推自己与刘备为英雄，令刘备震惊不已。
公元199年，河北袁绍夺占幽州，公孙瓒兵败自尽。自此袁绍已独占四州，与曹操隔黄河对峙。而一败涂地的袁术欲与袁绍联合，刘备趁机请命出兵，在路上截杀了袁术，顺势夺回了徐州。曹操大军来袭，刘备惨败，独自一人来河北投了袁绍。关羽在下邳死守，曹操爱惜关羽，派张辽说降。关羽与曹操约法三章而降。此时，江东孙策在行猎时遇刺，将基业传给弟弟孙权后死去。17岁的孙权在张昭、周瑜的辅助下继承了江东大业，他将对面极为艰难的考验。
官渡之战·北方一统(第30至33回)
公元200年，袁绍率军南下，与曹操大军交战于白马。曹操派关羽出战，大破袁军，袁绍大将颜良、文丑在交战中阵亡。关羽斩颜良立功，授汉寿亭侯。然而经此一役，关羽也得知了刘备的下落，挂印封金离开曹营，前往河北寻兄。一路饱经磨难，几经辗转最终与刘备、张飞在古城相会。千里独行、五关斩将的故事成为了千古传奇。因公孙瓒兵败，赵云苦寻心中的明主刘备，也终于在古城得偿心愿。
江东孙权继位后，拒绝相助袁绍与曹操交战。袁绍愤怒，起重兵与曹操对峙于官渡。交战中曹操失利，却苦苦坚持，以待战机。袁绍营中，谋士许攸多献战策却不被采纳，知袁绍将败而来投曹操，并提供了重要情报以助曹操偷袭乌巢。终于，曹操在乌巢打了一场翻身之战。一夜大火，袁军势力大衰，此后屡战屡败。公元202年，袁绍病死，长子袁谭与三子袁尚为夺袁绍基业而互相攻伐，河北陷入了二袁相争的局面。在郭嘉的建议下，曹操出奇兵远征大漠，收复了乌桓，最后在辽东苦寒之地剿杀了二袁，辽东公孙康归降，北方尽属曹操。然而归途中，郭嘉因水土不服而病故，又令曹操痛心不已。
